Background…
The Speech and Hearing Center, Inc. is a
not-for-profit, 501 (C) (3), agency serving northeast Florida
and southeast Georgia. We provide high-quality audiology and
speech/language pathology services to the community. Our operating
expenses are defrayed by revenue for services, the sale of
hearing aids, grants (United Way of Northeast Florida, City
of Jacksonville, Jessie Ball duPont Fund, Sertoma) and donations.
The Center is a member of the National Association
of Speech and Hearing Centers.

Purpose:
This fee scale would apply to any individual who is uninsured
or underinsured for the purpose of purchasing any hearing aid we
dispense.
The fee scale does not apply to out-of-warranty repairs.
Income eligible individuals who could reduce their personal out-of-pocket
expense by purchasing a hearing aid outright instead of using their
insurance coverage may elect to do so.
Individuals cannot combine insurance coverage and use the sliding
fee scale.
Eligibility:
To verify eligibility, we would require
a copy of the individual’s
latest tax return. (If you do not file an income tax return, please
provide an explanation.)
A Medicaid card, with current eligibility,
could serve as proof to obtain the discount for a hearing aid
purchase. However, a copy of the same individual’s latest
tax return would be necessary to establish eligibility for a
greater discount.

Definitions:
Uninsured – The individual who has
no insurance coverage whatsoever.
Underinsured – The individual who
may have some type of insurance, but whose coverage does not
include the purchase of hearing aids.
